ANKLE-BRACHIAL INDEX STUDIES:

BECAUSE PREVENTION IS OUR BEST DEFENSE, WE ACTIVELY SCREEN AT-RISK PATIENTS FOR PERIPHERAL ARTERIAL DISEASE VIA ANKLE-BRACHIAL INDEX STUDIES

At NSIM, we take the risk of heart disease very seriously. That is why we conduct ankle-brachial index (ABI) studies for our patients.

The ABI measures blood pressure at the ankle and in the arm while a patient is at rest. These measurements are then repeated at both sites after 5 minutes of walking on a treadmill. This test is administered to screen for peripheral arterial disease of the legs (also called PAD). This is important because PAD is linked to a higher risk of heart attack or stroke.

This test can also tell us how well a treatment is working for a patient (such as a medical treatment, an exercise program, angioplasty, or surgery).

CHOLESTEROL MANAGEMENT PROGRAM:

NSIM CHOLESTEROL MANAGEMENT PROGRAM PROVIDES TAILORED EDUCATIONAL SUPPORT TO HELP PATIENTS TAKE CONTROL OF THEIR CHOLESTEROL

High cholesterol—also known as hypercholesterolemia—is a major risk factor for heart disease and stroke. It’s imperative that patients with high cholesterol gain control of their cholesterol levels in order to improve their overall health and well-being. The good news is most patients can lower their cholesterol with the help of a cholesterol-lowering diet, exercise program, weight loss program and/or medication.

Many factors—including poor diet, inactivity, weight gain, stress and genetics—can contribute to the development of high cholesterol. Therefore, the NSIM Cholesterol Management Program looks at each patient individually. By partnering with our patients, we help tailor a specific lifestyle modification plan that enables each patient to gain control over cholesterol.

DIABETES MANAGEMENT PROGRAM:

NSIM Diabetes MANAGEMENT Program provides the highest level of INFORMATION to Diabetic patients so they can provide better care for themselves

Patients with diabetes often feel overwhelmed and fearful about their disease. However, patients can learn to take control of their diabetes and enjoy a higher quality of life with proper education on nutrition, medication, exercise and blood sugar monitoring.

Under the direction of Jorge L Gardyn, MD, FACP the NSIM diabetes management program helps patients learn how to effectively manage their disease with better lifestyle choices to improve their clinical outcomes.

HYPERTENSION MANAGEMENT PROGRAM:

NSIM HYPERTENSION MANAGEMENT PROGRAM PROVIDES AN AGGRESSIVE COUNSELING AND TREATMENT PROGRAM TO HELP PATIENTS REACH THEIR BLOOD PRESSURE GOALS

Hypertension, also referred to as high blood pressure, can lead to stroke, heart attack, heart failure, or kidney disease. At NSIM, the goal of hypertension treatment is to lower high blood pressure to protect important organs—such as the brain, heart, and kidneys—from damage.

Treating high blood pressure involves lifestyle changes and possibly one (or a combination) of many different drug therapies. At NSIM we educate and counsel all of our hypertensive patients on the many different lifestyle modifications that can positively influence their blood pressure levels, such as eating a healthier diet, losing weight, quitting smoking, reducing sodium intake, reducing alcohol intake, and getting more exercise. We also educate on the many different therapeutic classes of drugs available to treat the disease when necessary.

After initial treatment, we actively monitor all of our hypertensive patients with regular follow-ups so that we can track their blood pressure levels. Even after our patients reach their blood pressure goal, we continue to see them regularly to ensure they stay on track.

NUTRITIONAL COUNSELING:

THE GOAL OF NUTRITIONAL COUNSELING AT NSIM IS TO HELP OUR PATIENTS MAKE AND MAINTAIN HEALTHY DIETARY CHANGES

When it comes to nutrition, the challenge lies not in making the initial dietary changes, but in maintaining them over the long term. That’s why at NSIM we understand nutrition counseling is an ongoing process that can take months or even years. Our nutrition counselors not only provide information, educational materials, and support to our patients but we follow-up to help our patients maintain the dietary changes they’ve chosen to make.

Nutritional counseling at NSIM starts with an initial dietary assessment and interview which provides the basis for identifying behaviors that need to be changed. We work together with our patients to identify areas where change is needed, prioritize those changes, and then problem-solve as to how to best make those changes. In follow-up nutrition counseling sessions, we analyze food records and problem-solve behaviors that are especially difficult to change. Follow-up counseling also allows for the opportunity to reevaluate goals as well as the strategies for achieving those goals.

When it comes to Chest X-Rays, we are dedicated to the safe and efficient use of advanced imaging to help improve the health of the patients we serve. So to better meet the needs of our patients, we utilize digital imaging that eliminates the need for film and assists to expedite the processing of your exam and treatment. This enables us to offer our patients same day radiologic services.

Patients can schedule their chest x-rays and have them completed in the same day as the original appointment with their doctor.

VACCINATION PROGRAM:

THE NSIM VACCINATION PROGRAM PROVIDES ADULT IMMUNIZATIONS RECOMMENDED BY THE CENTERS FOR DISEASE CONTROL (CDC)

Vaccinations, also called immunizations, trigger the immune system to prevent serious, life-threatening diseases by teaching your body how to defend itself when germs such as viruses or bacteria invade it. Vaccinations expose patients to a very small, very safe amount of a virus or bacteria that has been weakened or killed. The immune system then learns to recognize and attack the infection if you are exposed to it. As a result, you will either not become ill or have a milder infection. This is a natural way to deal with infectious diseases.

At NSIM, we recognize that today we live in a global community. As such, we are exposed to more diseases from other countries than ever before—even without travelling. The CDC recommends adult immunizations including tetanus booster shots, the flu shot, hepatitis A and B vaccines, the pneumococcal vaccine, MMR, and immunizations for chickenpox and meningitis. We follow the CDC protocol to ensure that our patients receive the greatest protection possible when it comes to infectious diseases.

VENOUS, PERIPHERAL ARTERIAL AND CAROTID DOPPLER STUDIES:

AT NSIM, WE BELIEVE THAT METHODICAL SCREENING THROUGH NONINVASIVE PROCEDURES
PROVIDES THE BEST COMPREHENSIVE PATIENT CARE

Doppler ultrasound studies are noninvasive tests that can measure a patient’s blood flow and blood pressure by bouncing high-frequency sound waves off circulating red blood cells. These studies can estimate how fast blood flows by measuring the rate of change in its frequency.

At NSIM, we use Doppler ultrasound studies to help screen and diagnose many potentially
serious conditions, including:
• Blood clots
• Poorly functioning valves in your leg veins, which can cause blood or other fluids to pool in your legs
  (venous insufficiency)
• Heart valve defects and congenital heart disease
• A blocked artery (arterial occlusion)
• Decreased blood circulation into your legs (peripheral artery disease)
• Bulging arteries (aneurysms)
• Narrowing of an artery, such as those in your neck (carotid artery stenosis)
